Quick answer
An individualized plan for employment (IPE) must be reviewed at least annually. Substantive changes to the employment outcome, services, or providers require an amendment as necessary. The amendment takes effect only after the required agreement and signatures.
What to do
Identify exactly what changed since the signed plan.
Compare the existing wording with the requested wording.
Explain the new need or evidence and ask what further information is necessary.
Check both signatures, obtain a copy, and confirm how each approved change will be implemented.
Limits and deadlines
A meeting, email discussion, or completed assessment is not automatically an effective amendment.
An annual review does not guarantee renewal of every service. Ask for reasons and review rights when a service decision is disputed. State appeal deadlines are separate.
