Before you begin

Organize questions about an assessment without treating completion, disagreement, or a score as proof of adequacy or inadequacy.

Have these ready

  • Obtain the assessment report and, where available, its referral questions or scope.
  • Keep the original report unchanged.

Preparation only. Creating, downloading, or printing this worksheet does not file, serve, or send anything, prove agency receipt, or extend a deadline. Keep a separate record of any actual submission.

  • This is an educational review organizer, not a clinical evaluation or expert opinion.
  • An assessment being completed does not alone establish that it answers the referral question. A missing detail may require clarification before drawing a conclusion.

How to use it

  1. Write the employment-related question the assessment was intended to address.

  2. Locate the methods, information considered, supports provided, findings, and limitations.

  3. Separate a reported fact from an interpretation or a question you cannot yet answer.

  4. Request a specific clarification, correction, additional information, or review, with page references.

Fictional example

An assessment describes keyboard speed but does not explain whether the learner used the adaptive keyboard being considered. The learner records the relevant page and asks how the testing conditions affect the recommendation.
